Antioxidant Serum After Fractional CO2 Acne Scar Treatment in 2026: Sample Size and Evidence Limits for People With Boxcar Acne Scars

For boxcar scars, broad pits with sharp vertical walls, antioxidant serum after fractional CO2 improved early healing but lasting clearance is unproven. Trials are small, single-center and mix scar types, so findings support recovery care rather than guaranteed boxcar removal.

Fractional CO2 treatment creates tiny wound columns to trigger skin remodeling. Serum applied right after aims to calm inflammation and support barrier repair. For boxcar-prone skin, healing quality matters because redness and dark marks can linger after resurfacing.

What the early healing studies show

A January 2026 randomized, investigator-blinded split-face trial in Chinese adults aged 18-50 with moderate-to-severe atrophic scars tested CE Ferulic against saline after ablative fractional CO2. The serum side showed better wound healing, less erythema and melanin, and stronger barrier preservation, according to the Journal of Cosmetic Dermatology in January 2026 split-face results. The design compared two sides of the same face, which controls for individual healing differences.

An earlier double-blinded prospective single-center split-face trial treated 15 healthy adults aged 30-55, with serum on one cheek and vehicle on the other within 2 minutes after fractional ablative CO2 resurfacing. The treated side had less edema and erythema plus faster healing, as reported by MedEsthetics on the Waibel et al. study in laser-assisted antioxidant delivery report. Both trials focus on short-term recovery signs, not long-term boxcar clearance.

How small are the underlying trials

A 2026 meta-analysis of 8 randomized and comparative trials of fractional CO2 laser versus microneedling-radiofrequency found individual samples of only 12 to 50 participants across India, China, Pakistan, Iraq and Denmark, according to Wiley in the Journal of Cosmetic Dermatology in fractional CO2 versus microneedling-radiofrequency meta-analysis. Small samples make estimates unstable and subgroup claims fragile. They also limit study of different skin tones and scar depths.

Most fractional CO2-plus-adjunct acne-scar trials are small, single-center, short-follow-up and mix scar types. No large boxcar-only randomized trial establishes durable boxcar-specific clearance rates or recurrence, a limit authors routinely note when calling for multicenter validation, as disclosed in a 2023 Scientific Reports example. A 2016 Cochrane review of 24 trials likewise found only limited support for fractional laser and several other acne-scar options.

How do boxcar scars respond differently

Rolling and superficial boxcar scars tend to respond best to fractional CO2 monotherapy, while deep pitted and ice-pick scars respond least, according to Journal of Cosmetic Dermatology studies summarized in 2014 and 2025. A 2025 CO2-plus-poly-L-lactic-acid study noted particular benefit for rolling and boxcar scars but explicitly flagged limited sample size. Scar depth, tethering and subtype mix therefore shape results more than average improvement suggests.

A 2025 systematic review including 7 fractional CO2 studies in Biomedicines concluded therapy is effective and generally safe for acne scars. It also found transient erythema, edema and post-inflammatory hyperpigmentation were common and resolved within weeks. For boxcar patients, that short-term course affects downtime planning and pigment monitoring.

What to discuss before treatment

Clinicians individualize laser depth and often stage combinations because fractional laser alone has restricted penetration for deep or tethered scars and pigment risk persists, according to a 2025 Lasers in Medical Science review. Options discussed in 2026 include subcision, TCA CROSS, RF microneedling or punch procedures plus strict photoprotection.

The goal is matching method to scar shape rather than repeating one laser pass. Bring photos showing prior healing and dark marks, plus a list of pigment-prone reactions. Confirm exact aftercare steps and review timing before booking.

  • Ask for scar mapping by subtype, depth and tethering
  • Ask why CO2, combination or staging fits your boxcar pattern
  • Ask about pigment risk, downtime, sun protection and follow-up timing
  • Ask what healing support is planned for redness and barrier care
