Deep painful breakouts, often called cystic acne, happen when oil, dead skin, bacteria, and swelling team up deep under your skin. These are not your usual pimples; they form big, tender lumps that hurt to touch and can leave scars if not handled right.

It all starts inside your pores. Your skin makes oil, called sebum, to stay soft and protected. When glands pump out too much sebum, it mixes with dead skin cells and clogs the pore. Bacteria like Cutibacterium acnes, which live on your skin, get trapped in there and start to grow. This sparks a big inflammatory reaction that pushes down into deeper skin layers, creating a cyst filled with pus and swelling.[1][2][3][5]

Hormones play a huge role in kicking this off. During puberty, monthly cycles, pregnancy, or stress, hormone shifts make oil glands work overtime. Women often see these breakouts along the jawline, which can signal issues like polycystic ovarian syndrome or changes from birth control.[1][2][6] Genetics matter too; if your family has bad acne, you are more likely to get it.[1][3]

Everyday habits can make things worse. Eating lots of sugar, white carbs, dairy, or whey protein spikes insulin and oil production for some people. Stress raises hormones that boost oil. Poor sleep, heavy makeup, sweaty workouts, or using pore-clogging skincare add fuel to the fire. Even over-washing your face strips the skin barrier, leading to more oil and irritation.[1][2][3][6][7]

Not every painful bump is acne. Staph infections like MRSA start as red, warm bumps that turn into deep boils and might need antibiotics. Folliculitis from shaving or sweat inflames hair follicles. If bumps itch, cluster evenly, or ignore acne treatments, it could be something else like rosacea or perioral dermatitis.[2][4]

These breakouts brew for weeks or months before showing up, so spotting early triggers helps. They hit oily spots like the face, jaw, chest, or back hardest.[1][2][5]
