Topical Hormonal Acne Treatments in a 2026 Review: What the Findings Mean for Adults With Persistent Acne

The January 2026 review in the Journal of Clinical and Aesthetic Dermatology finds topical hormonal acne treatments block androgen signals directly in the skin, with clascoterone 1% cream as the established option. For adults with persistent acne, that means less oil and inflammation without taking systemic hormones.

Topical hormonal means a cream that acts on hormone receptors in skin. Systemic hormonal pills travel through the whole body, while these creams target sebaceous glands where breakouts start. Persistent adult acne often continues from the teen years rather than starting late.

How these creams calm hormonal breakouts

Clascoterone competes with androgens and DHT at androgen receptors in skin. That action lowers sebum production and calms inflammation. According to the MDPI Pharmaceuticals narrative review by Łyko in 2026, the drug then rapidly changes into inactive cortexolone.

That rapid breakdown matters for daily use. Less active drug reaches the bloodstream, so systemic hormonal exposure stays low. The effect stays centered on treated skin.

How well does clascoterone work?

Two identical Phase 3 trials in 1,440 patients showed week-12 treatment success in 18.4% and 20.3% on clascoterone versus 9.0% and 6.5% on vehicle. Both trials also showed significant drops in inflammatory and noninflammatory lesions.

A 52-week facial-sebum study added direct oil measurements. According to Healio's report on the Maui Derm Hawaii 2026 poster, mean sebumeter values fell from 115.9 to 61.5, a 47% reduction (Healio's report on the Maui Derm Hawaii 2026 poster). The same study reported 73% fewer inflammatory lesions, 61% fewer noninflammatory lesions and 55% lower IGA score.

Who benefits most?

Clascoterone 1% cream, Winlevi, was FDA-approved August 26, 2020 for acne vulgaris in patients 12 and older. According to the FDA approval history on Drugs.com, it was the first topical androgen-receptor inhibitor and first new acne mechanism in nearly 40 years (the FDA approval history on Drugs.com). Unlike systemic spironolactone and combined oral contraceptives generally limited to women because of feminizing risks, topical clascoterone is usable by both women and men.

That makes it useful for patients avoiding systemic hormones. Adult female acne affects about 15-20% of adult women and is persistent rather than late-onset in about 80% of cases. Clinicians often pair clascoterone with a retinoid or benzoyl peroxide for stubborn acne. Practical steps readers can discuss with a clinician include:.

  • use twice daily as labeled for consistent receptor blockade
  • pair with a retinoid or benzoyl peroxide only when advised
  • skip cut, eczematous or sunburned skin

What are the safety limits?

At labeled twice-daily dosing, Phase 3 trials required no HPA-axis monitoring. The main effects were mild local redness and irritation. Transient subclinical HPA suppression occurred only under maximum-use conditions and normalized within about 4 weeks.

Head-to-head evidence remains limited. According to the EuroGuiDerm short version in the Journal of the European Academy of Dermatology and Venereology, hormonal therapy and off-label spironolactone are adjuncts for females with severe papulopustular or nodular disease, with limited comparisons for clascoterone versus established topicals (the EuroGuiDerm guideline short version). Do not apply clascoterone to cut, eczematous or sunburned skin. Ask your clinician about pregnancy and breastfeeding because safety data are lacking.
