Dermatologist Acne Prescription

A dermatologist acne prescription is a prescription-strength topical or oral drug chosen after a dermatologist evaluates acne severity. It targets clogged hair follicles, excess sebum, Cutibacterium acnes bacteria, and inflammation to heal lesions and prevent scarring. Topicals usually come first, and oral drugs are added for moderate-to-severe disease. The National Institute of Arthritis and Musculoskeletal and Skin Diseases explains that treatment aims at clogged follicles, excess sebum, acne bacteria, and inflammation to heal lesions and prevent scarring NIAMS acne treatment overview.

Which prescription topicals clear pores and bacteria?

Topical benzoyl peroxide lowers acne bacteria on the skin. Topical retinoids unclog pores and reduce inflammation. A retinoid is a vitamin-A-derived cream or gel applied to the skin. The American Academy of Dermatology 2024 guidelines strongly recommend benzoyl peroxide plus the retinoids adapalene, tretinoin, tazarotene, and trifarotene AAD guideline update.

The same guidelines strongly recommend topical antibiotics and fixed-dose combinations. Those combinations pair a retinoid with benzoyl peroxide or an antibiotic with benzoyl peroxide. Fixed combinations simplify routines and attack bacteria and clogged pores together. A dermatologist selects the strength and vehicle based on severity, skin sensitivity, and prior response.

When is an oral antibiotic needed?

Oral doxycycline is an option for appropriate moderate-to-severe acne when topicals alone are not enough. Patients usually keep using prescription topicals while taking the oral drug and after stopping it.

Dermatologists may also inject a large painful nodule with a corticosteroid for faster relief. Systemic antibiotics are limited when possible. The American Academy of Dermatology advises using oral and topical antibiotics together with benzoyl peroxide and reassessing after about 3-4 months to reduce resistance and complications.

  • Ask what topicals to continue during and after the antibiotic course
  • Ask when the 3-4 month reassessment will occur
  • Ask what maintenance topical will remain after antibiotics stop

What if acne is severe or scarring?

Oral isotretinoin is reserved for severe recalcitrant nodular acne. Recalcitrant means acne that has not improved with other treatment. Nodular means large, deep, painful lumps under the skin. Brands include Absorica, Claravis, Amnesteem, Myorisan, and Zenatane. The American Academy of Dermatology strongly recommends isotretinoin for severe or scarring disease.

The U.S. Food and Drug Administration regulates it as a prescription-only drug. Isotretinoin can cause severe birth defects. Prescribers, pharmacists, and patients who can become pregnant must follow the federal iPLEDGE program. That program requires registration, pregnancy testing, contraception or abstinence attestations, and 30-day supply limits.

What helps hormone-influenced breakouts?

For hormone-influenced acne, the American Academy of Dermatology conditionally recommends combined oral contraceptives and spironolactone for appropriate patients. These drugs act throughout the body rather than only where applied. Clascoterone 1 percent cream, sold as Winlevi, works differently.

It is an androgen-receptor inhibitor that blocks male-hormone signals in the skin. Cassiopea's announcement of the FDA approval notes twice-daily application for patients 12 and older after approval on Aug. 27, 2020 FDA approval announcement for Winlevi.
