The 2026 Oral Probiotic and Topical Secretome Acne Trial Protocol: Product-Claim Questions for People With Acne-Prone Skin

The 2026 protocol tests oral probiotics plus topical secretome as add-ons to standard care for mild-to-moderate acne vulgaris. For people with acne-prone skin, it gives a clear benchmark for judging ads that promise microbiome restoration or gut-skin balance. Secretome means a cell-free conditioned medium with secreted growth factors, cytokines, matrix proteins, exosomes and microvesicles. The National Institutes of Health systematic review defines it that way in the NIH conditioned-medium review, not as live stem cells.

What the 8-week protocol actually tests

The BMJ Open protocol randomises 64 patients with mild-to-moderate acne into four parallel arms. Each arm combines standard therapy with active or placebo oral probiotic and active or placebo topical secretome. Treatment runs for 8 weeks with assessments at baseline and after therapy.

Measures include acne severity, faecal microbiome changes and related clinical outcomes. BMJ Open publishes the full design in the acne add-on protocol. Because every group keeps standard care, the trial tests added benefit, not replacement. A product that cites this protocol should match that add-on use, acne severity range and 8-week course.

What probiotic results show so far

A 12-week randomised, double-blind, placebo-controlled trial in ages 12 to 30 reported improvement on the Acne Global Severity Scale in 20 of 40 probiotic recipients versus 10 of 34 placebo recipients. The product combined Lacticaseibacillus rhamnosus CECT 30031 with Arthrospira platensis, according to the Acta Dermato-Venereologica trial record. A GRADE systematic review and meta-analysis of Lactobacillus-based randomised trials found a different pattern.

The Journal of Cosmetic Dermatology analysis found no significant reduction in inflammatory, non-inflammatory or total lesion counts versus placebo or benzoyl peroxide, with moderate-to-low certainty. For buyers, strain, dose and duration matter. A win with one strain combination does not prove that yoghurt, a different probiotic capsule or a shorter course will clear acne.

What secretome and exosome labels mean

A jar or serum labeled secretome, conditioned media or exosomes should contain cell-free signals, not living stem cells. The NIH review lists growth factors, cytokines, extracellular-matrix proteins, exosomes and microvesicles as the active mixture. Off-the-shelf claims face a regulatory limit.

Dermatology Times reporting on FDA actions states that no exosome product has completed FDA approval in the FDA exosome explainer, so therapeutic exosome use in the U.S. is lawful only under a formal Investigational New Drug application. That distinction helps at the shelf. Cosmetic language about glow or texture is different from a claim that a cream treats acne, repairs microbiome balance or works like the trial secretome.

How to question a claim before you buy

The Federal Trade Commission requires health-benefit claims to rest on competent and reliable scientific evidence in the FTC health-products guidance, generally randomised controlled human trials judged by relevant experts. Acne-prone buyers can hold sellers to that human-trial standard. Send the seller one short request: share the published human trial for this formula and dose for mild-to-moderate acne.

  • Ask for the human randomised trial behind the exact product, dose and use period.
  • Check whether controls received placebo or standard care, and whether acne severity matches yours.
  • Look for lesion counts or severity scores, not only satisfaction ratings or before-and-after photos.
  • Be cautious when a seller cites the 2026 protocol alone, because a protocol describes planned work rather than results.
