What the trial compared
Researchers randomly assigned 80 people to 10 mg isotretinoin plus 1 g fish oil daily or 10 mg isotretinoin plus placebo daily for 3 months, according to the JAAD abstract JAAD abstract. The study was randomized, double-masked, and controlled.
Double-masked means patients and evaluators did not know who received fish oil. Non-inferiority means the combination had to prove it was not meaningfully worse than isotretinoin alone, not that it was better.
Who was in the study and who was left out
The studied group was narrow. All participants were 18 to 25 and had moderate-to-severe acne vulgaris.
That cutoff matters for persistent acne. An adult over 25 cannot assume the same acne response or tolerability because that age group was excluded by design.

It also noted smaller drops in skin-hydration readings and smaller rises in triglycerides in the fish-oil group. In plain terms, acne improvement held while dryness and lipid shifts looked milder. Corneometer readings are device measures of surface hydration, not a promise of less peeling.
What if you have persistent acne after 25?
The trial does not establish maintenance therapy for persistent acne. The authors noted that IGA-based entry may have affected the noninferiority analysis and that three months limits long-term conclusions.
