Antioxidant Serum After Fractional CO2 Acne Scar Treatment in 2026: Sample Size and Evidence Limits for People With Rolling Acne Scars

Antioxidant serum after fractional CO2 laser speeds early healing, but 2026 proof comes from only 64 evaluable adults followed for 14 days. People with rolling acne scars — broad shallow dips with sloping edges that look wavy — cannot read that average atrophic-scar result as rolling-scar proof. Fractional CO2 laser is an ablative treatment that creates tiny wounds to trigger repair. In the trial, vitamin C plus vitamin E plus ferulic acid serum went on one cheek and saline on the other right after treatment.

What did the 2026 trial test?

According to the U.S. National Library of Medicine / PubMed, investigators ran a January 2026 randomized, investigator-blinded, split-face trial in Chinese adults ages 18-50 with moderate-to-severe atrophic scars the PubMed record. One cheek received antioxidant serum for 14 days right after ablative fractional CO2 laser.

The other cheek received normal saline. According to NIH / PubMed Central, the serum side healed faster and showed lower redness and melanin-index measures plus better hydration than saline the PubMed Central full text. That supports serum as post-procedure recovery care. It does not prove permanent scar removal.

How small and short is the evidence?

According to Wiley / Journal of Cosmetic Dermatology, researchers recruited 66 patients and split them 1:1 into once-daily and twice-daily serum groups to cover dropouts the Journal of Cosmetic Dermatology report. Summaries report 64 evaluable adults. The sample is small and short-term.

The Archyde summary of the Wiley trial design notes a 14-day topical period with investigator-blinded but not double-blind checks at one clinic. Those checks capture early redness, pigmentation, and hydration. They do not track long-term scar remodeling or recurrence.

Why can't rolling scars use the average?

The trial registration ChiCTR2300078214 enrolled only moderate-to-severe atrophic facial scars in China, as listed by the U.S. National Library of Medicine / PubMed. It did not test rolling scars alone.

It also did not test other ages, populations, or skin types. Separate fractional CO2 work reported by the Journal of Cutaneous and Aesthetic Surgery found rolling and superficial boxcar scars respond best while deep pitted and icepick scars respond least the scar-subtype resurfacing study. A person with purely rolling scars sits at one end of that range. An average atrophic-scar score can hide that difference.

What should you ask before laser?

The Healio dermatology consensus advises clinicians to plan multiple sessions and match add-ons to scar subtype. For tethered rolling scars, that often means subcision to release fibrous bands.

Antioxidant serum fits as healing support to cut post-laser problems, not as a scar eraser. Bring photos of your scars in different light and ask which subtype dominates. Book only when the plan names sessions, add-ons, and aftercare.

  • Ask which subtype dominates your cheeks: rolling, boxcar, or icepick
  • Ask how many laser sessions fit your subtype and what add-on treats tethering
  • Ask what aftercare covers redness, dark marks, dryness, and sun protection
