Antioxidant serum after fractional CO2 laser has evidence from 64 adults, but it supports faster healing — not erasing icepick scars. Icepick scars are narrow, deep V-shaped pits, and the trial enrolled mixed moderate-to-severe atrophic scars rather than icepick-only disease.
Ablative fractional CO2 laser removes tiny columns of tissue to trigger repair. In the trial, staff applied 15% vitamin C/vitamin E/ferulic acid to one facial half and saline to the other for 14 days. Healing, redness, pigment, hydration and barrier function were then compared side to side.

What the 64-person trial tested
Wiley/Journal of Cosmetic Dermatology published the 64-adult randomized, investigator-blinded split-face trial in adults aged 18-50 with moderate-to-severe atrophic acne scars the Wiley trial report. One cheek received CE Ferulic and the other received normal saline. PubMed/PMC details the ChiCTR2300078214 protocol and its 14-day application schedule with wound, color, hydration and barrier measures the PMC trial record.
The design controls well for person-to-person healing differences. It does not control as well for formula effects because saline is not a matching vehicle. The CE Ferulic side healed faster and held hydration and barrier function better. Redness and melanin readings were lower than on the saline side.
What improved after the laser
The main gains were recovery measures, not scar depth. Treated halves showed quicker wound closure, less erythema, lower melanin levels and better moisture retention. That pattern points to calmer healing and lower post-inflammatory hyperpigmentation risk. A separate 51-patient split-face test adds context.
It applied CE Ferulic plus resveratrol BE twice daily for 14 days after ablative fractional CO2. The antioxidant sides again showed significantly less erythema and hyperpigmentation than saline. Neither result means pits filled in. The trials measured short-term surface recovery over two weeks. Scar remodeling takes months and needs separate measurement.
Why icepick scars respond less to laser alone
Rolling, boxcar and icepick scars differ in shape and depth. Rolling scars are broad and shallow, boxcar scars have sharper walls, and icepick tracts are narrow and steeply epithelialized. Surface resurfacing reaches the first two more directly. The Journal of Clinical Medicine scar review found fractional CO2 improves rolling atrophic scars more than boxcar and icepick scars the scar-subtype review.
Narrow epithelialized tracts resist surface treatment. Fractional-laser monotherapy is therefore unlikely to resolve deep pitted scars. Dermatology guidance directs icepick scars primarily to TCA CROSS or punch excision and elevation. Fractional ablative laser serves as an adjunct for surrounding texture. It does not stand alone as closure for icepick tracts.
Evidence limits that affect your decision
The 64-patient result is short-term and single-center in China. Follow-up covered healing days, not long-term scar scores. Icepick-only readers should weigh that gap heavily. Blinding and control were partial.
Investigators were blinded, but patients were not double-blinded. Saline matched the application schedule, not the serum base. Enrollment was mixed atrophic disease. The paper did not isolate icepick cases or report outcomes by scar subtype. People whose scars are mostly icepick cannot read the average benefit as their likely benefit.
What to do if you have mostly icepick scars
Expect two separate goals: faster post-laser recovery and actual pit treatment. Antioxidant serum may help the first goal through less redness and better barrier support. Pit closure still usually needs a subtype-targeted method plus strict photoprotection, confirmed with a dermatologist.
Use the visit to get a scar map, not a single-device plan: Bring photos of prior prolonged redness or dark marks after injury or procedures. Ask whether your pigment risk changes product choice or laser settings. Leave with a written sequence for pits first, texture second and maintenance last.
- ask which scars are icepick, boxcar, rolling or mixed
- ask whether TCA CROSS, punch methods or adjunct laser fit each type
- ask what post-procedure product, timing and sun protection the clinic uses
- ask how progress will be measured and when to reassess
