What Causes Breakouts With Hormonal IUDs
Hormonal IUDs, like Mirena, release a hormone called levonorgestrel into the uterus to prevent pregnancy. This hormone can lead to skin breakouts, known as acne, in some users. The main reason is that levonorgestrel acts like progesterone, which boosts androgen hormones in the body. Androgens tell the skin’s oil glands, called sebaceous glands, to make more oil. Too much oil clogs pores and causes pimples.
This effect often starts in the first few months after insertion. Studies show acne happens in about 7 percent of Mirena users. The extra oil makes skin oily and traps dirt or dead skin cells, leading to blackheads, whiteheads, or worse breakouts. Not everyone gets acne, but those sensitive to hormones are more likely to notice it.
The hormone stays mostly local in the uterus, thickening mucus to block sperm and thinning the uterine lining. But small amounts can spread and affect the whole body, including skin. Other side effects like breast tenderness or mood changes come from the same hormone shift. Acne usually gets better after three to six months as the body adjusts.
Breakouts from hormonal IUDs differ from those caused by other birth control like implants, which use a different hormone called etonogestrel. That one also ramps up oil production but can cause more severe acne in some cases. With IUDs, the acne link comes from how levonorgestrel increases sebaceous gland activity, as noted in medical research.
