Fish Oil With Low-Dose Isotretinoin in a 2026 Randomized Trial: Who Was Studied and Who Was Not for Adults With Persistent Acne

The 2026 trial studied adults ages 18 to 25 with moderate-to-severe acne, not adults over 25 with persistent acne. It tested 10 mg isotretinoin daily with 1 g fish oil daily versus isotretinoin alone for three months.

Isotretinoin is a prescription drug that lowers skin oil and clears lesions. In this trial, moderate-to-severe meant an Investigator's Global Assessment score of 3 or higher, according to the U.S. National Library of Medicine PubMed record PubMed record.

What the trial compared

Researchers randomly assigned 80 people to 10 mg isotretinoin plus 1 g fish oil daily or 10 mg isotretinoin plus placebo daily for 3 months, according to the JAAD abstract JAAD abstract. The study was randomized, double-masked, and controlled.

Double-masked means patients and evaluators did not know who received fish oil. Non-inferiority means the combination had to prove it was not meaningfully worse than isotretinoin alone, not that it was better.

Who was in the study and who was left out

The studied group was narrow. All participants were 18 to 25 and had moderate-to-severe acne vulgaris.

That cutoff matters for persistent acne. An adult over 25 cannot assume the same acne response or tolerability because that age group was excluded by design.

  • Ages 18 to 25 only; adults in their 30s and 40s were not enrolled
  • Investigator's Global Assessment score of 3 or higher only; mild acne was not studied
  • Ages under 18 were not studied
  • Only 10 mg isotretinoin daily for 3 months was tested; other doses and longer courses were not tested

How did acne, hydration, and triglycerides change?

Total acne-count reduction was similar between groups, with a between-group mean difference of -6.0, according to the MIMS Malaysia report on the trial MIMS Malaysia report. The report described the combination as noninferior to isotretinoin alone.

It also noted smaller drops in skin-hydration readings and smaller rises in triglycerides in the fish-oil group. In plain terms, acne improvement held while dryness and lipid shifts looked milder. Corneometer readings are device measures of surface hydration, not a promise of less peeling.

What if you have persistent acne after 25?

The trial does not establish maintenance therapy for persistent acne. The authors noted that IGA-based entry may have affected the noninferiority analysis and that three months limits long-term conclusions.

Use the trial as a discussion aid, not a buying guide. Bring your exact age band, acne severity, pregnancy status, lipid history, current isotretinoin dose, and any fish-oil or supplement use to a dermatology visit and ask whether any part of this 10-mg, 3-month approach fits your case.
