Pretreatment Pregnancy Testing Under iPLEDGE in 2026: What the FDA Update Means for Adult Acne Patients

Pretreatment pregnancy testing under iPLEDGE still must happen in a medical setting in 2026 for adult acne patients starting isotretinoin. The FDA update keeps those first tests in person while allowing later tests during and after treatment to happen at home with prescriber approval.

iPLEDGE is the shared safety program for isotretinoin, a drug for severe nodular acne that can cause severe birth defects. The FDA approved changes on Feb. 9, 2026 to reduce burden on patients, prescribers and pharmacies while keeping safe use, according to the FDA approval notice reported via Drugs.com the FDA approval notice.

What must happen before your first dose?

All pregnancy testing required before you take a first isotretinoin dose for a course of treatment must be completed in a medical setting. That setting includes a prescriber office, clinic or laboratory, according to the FDA medical organizations letter FDA communication letter.

The first pretreatment screening test is obtained when you and your prescriber decide to use isotretinoin. For an adult starting a first course for stubborn nodular acne, that means scheduling the blood or urine test at the clinic, not at home.

Which tests can move to home?

For patients who can become pregnant, prescribers may allow pregnancy testing during and after isotretinoin treatment to be completed at home. The National Community Pharmacists Association describes this as a prescriber option, not an automatic change.

That option does not extend to pretreatment testing. An adult who already finished one course and plans another course still needs in-person tests before the next first dose.

What if you miss the pickup window?

The 19-day lockout after missing the 7-day prescription pickup window was eliminated. A repeat pregnancy test can now be done immediately with no added waiting period, according to JDNPPA acne news.

That change lowers the penalty for a timing slip. If your pharmacy cannot dispense within seven days, ask your prescriber promptly about retesting and a new prescription.

What should adult patients do now?

The approved changes were to take effect 180 days after approval, around Aug. 8-9, 2026, with enforcement discretion until then.

The FDA later delayed implementation to Nov. 15, 2026 for system testing, according to the Lachman Consultants implementation update implementation update. Until your clinic confirms the new process is live: Call your prescriber before assuming home testing applies to you.

  • Plan all pretreatment tests as in-office visits
  • Ask whether follow-up tests can be home tests under your prescriber
  • Confirm pickup dates so you stay inside the 7-day window
