What Causes Acne That Leaves Long Lasting Redness

Acne that leaves long lasting redness happens mainly because of something called post inflammatory erythema, or PIE. This is when the skin stays red or pink for weeks, months, or even longer after pimples heal.

PIE starts with regular acne. Acne forms when hair follicles get clogged by oil, dead skin cells, and bacteria like Cutibacterium acnes, often called C. acnes. This bacterium grows in the oil and breaks it down into irritating fatty acids. These acids bother the skin lining inside the pores. The bacteria also wake up the immune system by turning on special sensors called toll like receptors on skin cells. This leads to a flood of inflammation chemicals, like interleukin 1 alpha, interleukin 8, and tumor necrosis factor alpha.

All this inflammation swells the skin and widens tiny blood vessels near the surface. The redness from acne comes from these dilated vessels. Even when the pimple goes away and the swelling fades, the blood vessels can stay stretched out. They show through the thin top layer of skin as red spots. In tougher cases, the body might grow new blood vessels during healing, which keeps the redness going.

The worse the acne inflammation, the stronger and longer the PIE tends to last. Deep cysts or long lasting pimples cause more damage to vessels, so the marks stick around. Things like hormones, stress, or too much oil can make acne worse first, leading to more PIE later. Genetics play a role too, since some people get hit harder by these reactions.

People sometimes mix up this redness with rosacea, but they differ. Rosacea brings steady flushing and vessel issues from triggers like sun, heat, or alcohol, without the clogged pores of acne. Acne PIE follows healed breakouts, while rosacea builds over time.
