Why Some Pimples Hurt But Never Surface

You have probably felt a sore spot under your skin that throbs but never pops up as a regular pimple. These are called blind pimples or cystic acne. They form deep inside the skin and cause more pain than the ones that show on top.[1]

Blind pimples start when your pores get blocked. Oil from your skin mixes with dead skin cells and traps bacteria deep down. This clog sits in the middle layer of your skin, called the dermis. The body fights the infection with swelling, which presses on nerves and blood vessels nearby. That pressure makes the area tender and painful, but since there is no path to the surface, no pus or head forms.[1]

Hormones play a big role in this. Androgens are hormones that tell your oil glands to make more sebum, the skin’s natural oil. When androgen levels rise, like before a woman’s period or during stress, glands overproduce oil. This leads to deeper clogs instead of shallow ones that turn into whiteheads or blackheads.[1][4]

These pimples often show up on the chin, jawline, or cheeks. They feel like hard, red lumps under the skin. Unlike surface pimples that drain and heal faster, blind ones build pressure with no release, so the hurt lasts longer.[1]

Rubbing or squeezing can make it worse. This pushes bacteria even deeper, turning a small issue into a bigger swollen cyst.[1]

Some blind pimples fade on their own if mild, but many stick around and grow. They can leave scars if not treated right.[1]
