Why Does Acne Appear After Hormonal Shifts

Acne often pops up after hormonal shifts because these changes make your skin produce too much oil, clogging pores and sparking breakouts. Hormones like androgens, which both men and women have, tell the sebaceous glands in your skin to ramp up oil, or sebum, production.[1][2] When sebum builds up, it mixes with dead skin cells and blocks hair follicles, trapping bacteria that cause swelling and pimples.[1][3]

These shifts happen at different life stages. During puberty, rising androgens kickstart oil overdrive, leading to teen acne on the forehead and cheeks.[5][6] In women, the menstrual cycle brings ups and downs: progesterone climbs before your period, boosting oil and causing spots on the chin, jawline, and lower cheeks about seven to ten days beforehand.[2][3] Pregnancy floods the body with hormones, making glands overactive and pores clogged, though it eases for some after birth.[1][3]

Menopause drops estrogen levels, letting androgens take over without balance, so skin gets drier yet prone to deep, painful cysts that resist usual treatments.[1][4] Conditions like polycystic ovary syndrome, or PCOS, raise androgens steadily, pairing acne with irregular periods, weight gain, and extra hair growth.[2][6] Stress adds fuel by releasing cortisol, which amps up oil and inflammation.[1][2]

Even medications or thyroid issues can tip hormones off kilter, worsening the cycle of excess oil, bacteria growth like Cutibacterium acnes, and immune reactions that inflame skin.[1][6] Unlike teen acne, adult hormonal breakouts tend to be cystic, slower to heal, and cluster on the lower face, neck, chest, or back.[1][6]
