Why Acne Leaves Marks Without Scarring

Acne often leaves behind flat dark spots or red marks on the skin even after the pimples heal, but these are not scars. They happen because your skin produces extra pigment as a natural defense during healing, changing the color without altering the skin’s texture.

When a pimple forms, it causes inflammation deep in the skin. This triggers special cells called melanocytes to make more melanin, the pigment that colors your skin. Your body does this to protect the area, much like it did for our ancestors against sun damage. The result is post-inflammatory hyperpigmentation, or PIH, which shows up as brown or black spots, especially on darker skin tones.

On lighter skin, you might see post-inflammatory erythema, or PIE, instead. These are pink or red marks from damaged tiny blood vessels under the skin, not extra pigment. Both PIH and PIE are flat and fade over time, unlike scars that create dents, ridges, or raised areas from actual tissue damage.

The worse the acne inflammation, like with deep cysts, the more likely these marks appear. Picking or squeezing pimples makes it worse by spreading inflammation. Sun exposure after healing can darken the spots even more, as UV rays boost melanin production.

These marks can last months or longer depending on your skin type and care. Darker skin tends to get more stubborn PIH, while lighter skin shows PIE more clearly. The good news is they are temporary and respond well to gentle treatments that calm inflammation and even out color.
