What Causes Uneven Skin Tone After Acne

After acne heals, many people notice dark spots or patches on their skin that make the tone look uneven. This happens because the skin produces extra melanin, the pigment that colors your skin, as a response to the inflammation from acne breakouts.[1][2]

When you get acne, the pimples or cysts cause swelling and irritation deep in the skin. Your body sees this as a threat and tells special cells called melanocytes to make more melanin to shield the area. This is a natural defense, like how skin tans in the sun, but it leaves behind flat, discolored marks instead of changing the skin’s texture.[1][4]

The worse the acne, the more likely you are to get these spots. For example, deep cystic acne triggers stronger inflammation, leading to darker patches than mild whiteheads or blackheads.[1] These marks are called post-inflammatory hyperpigmentation, or PIH. On lighter skin, they might look red or pink at first from dilated blood vessels, known as post-inflammatory erythema or PIE. On darker skin, they turn brown or black.[1]

People with medium to dark skin tones, like Fitzpatrick types III to VI, are more prone to PIH. Up to 65 percent of them may develop these spots after acne, compared to just 18 percent with lighter skin.[1] Genetics play a role too, making some folks more likely to overproduce melanin.[2][3]

Sun exposure makes it worse. UV rays from the sun boost melanin production even more, darkening the spots and slowing their fade. If you pick or squeeze pimples, that extra trauma ramps up inflammation and melanin, creating bigger uneven areas.[1][3][5]

Hormonal shifts can add to the problem. Things like birth control, pregnancy, or menstrual cycles change hormone levels, which signal the skin to produce uneven pigment, especially on top of acne damage.[2][3]

Certain medications, such as antibiotics, can also trigger more melanin in response to skin irritation from acne treatments.[2][6]

In short, uneven skin tone after acne comes from your skin’s overzealous healing process, fired up by inflammation, sun, hormones, and other triggers.
