Rolling acne scars are those wavy or undulating dips on the skin that make the surface look uneven, like gentle hills and valleys. They form mainly from severe, long-lasting acne that damages the skin deeply.

The process starts with inflammatory acne, like nodules or cysts, that go beyond the surface and harm the deeper layers. This inflammation destroys collagen, the protein that keeps skin firm and plump. When the skin tries to heal, it does not make enough good collagen, leaving weak spots.

What makes rolling scars different is the fibrous bands that develop under the skin. These tough bands of scar tissue stretch from the surface down to deeper fat layers. They act like anchors, pulling the skin downward at certain points. This tethering creates the rolling effect, especially noticeable on cheeks and jawlines.

Over time, things get worse. As we age, natural collagen and elastin break down more, making the dips deeper and the waves more obvious. Picking at pimples or repeated breakouts in the same spot speeds this up by causing ongoing damage.

Not every acne lesion leads to scars. They happen when the damage is bad enough to mess up healing completely. Too little collagen means the skin sinks instead of bouncing back smooth.
