Acne nodules are those deep, hard, painful lumps under the skin that feel like firm bumps you can press on but not squeeze out. They form when a clogged pore bursts and spills oil, dead skin cells, bacteria, and inflammation deep into the surrounding skin tissue[1][4][5].
It all starts with a pore getting blocked. Every pore has a sebaceous gland that makes sebum, the oil that keeps skin soft. When dead skin cells stick together instead of shedding normally, they mix with extra sebum and trap it inside the pore[1][3][4][7]. This creates a perfect spot for Cutibacterium acnes bacteria, which naturally live on skin, to grow out of control[1][4][5].
The body fights back by sending white blood cells called neutrophils to attack the bacteria. These cells release enzymes that break down the pore wall. Once it ruptures, the contents leak deep into the skin, causing a big inflammatory reaction that builds a nodule over weeks[1][4]. These nodules are bigger than 5 millimeters, red, and very tender because the swelling presses on nerves[1].
Several things make nodules more likely. Hormones top the list. Androgens, male hormones present in everyone, ramp up sebum production, especially during puberty, menstrual cycles, pregnancy, or conditions like PCOS[1][2][4][6]. Women often get them along the jawline due to hormonal shifts[2][4].
Genetics play a big role too. If your parents had severe acne, you are at higher risk because certain genes affect oil production and inflammation[1][3][4].
Age matters most in teens and early twenties when hormones surge, but adults over 30 can get them from ongoing hormone changes, stress, or birth control shifts[1][2][4].
Diet can trigger flares. Foods high in sugar like sodas and pastries, whey protein, and dairy raise insulin and hormones that boost oil[2][4]. Stress releases cortisol, which also stimulates glands[3][4][6].
Other factors include over-washing, which strips the skin barrier and causes more oil, or using irritating products[2][3]. Medications and strong immune responses to clogs can worsen it[3].
Nodules take 8 to 12 weeks to form deep down, so they appear suddenly after building silently[4]. They often leave scars if picked or untreated[1][4][6].
