Acne scarring on the body happens when severe acne damages deep layers of skin, and the body’s healing process creates uneven collagen, leaving behind pits, waves, or raised bumps. This is most common with cystic or nodular acne on areas like the back, chest, or shoulders, where inflammation goes deeper than surface pimples.

The main trigger is inflammation from deep acne lesions. Cysts and nodules burrow into the dermis, the skin’s structural layer, causing injury that disrupts normal healing. Your body rushes to repair it by making collagen, the protein that keeps skin firm. If it makes too little collagen, the skin sinks into atrophic scars like ice pick holes, boxcar craters with sharp edges, or rolling waves with soft slopes. Too much collagen leads to hypertrophic scars that rise above the skin, sometimes growing into keloids on the body.

Severity and duration of breakouts play a big role. Long-lasting or repeated inflammation in the same spot worsens tissue damage, especially on oilier body areas prone to acne. Genetics influence this too, as some people produce more oil or heal with more scarring. Oily skin on the back or chest often traps debris, stretching pores and adding to rough texture alongside scars.

Picking or squeezing pimples adds extra harm. This pushes bacteria deeper, ramps up inflammation, and tears collagen fibers directly, turning a simple breakout into a permanent mark. Delaying treatment lets inflammation linger, increasing scar risk. Harsh habits like over-scrubbing or using rough exfoliants irritate skin further, making matters worse.

Sun exposure darkens scars and breaks down collagen, slowing fade on exposed body parts. Age factors in as older skin loses elasticity, making rolling scars more visible over time.
