Why Do Some Acne Scars Become Depressed
Acne scars turn depressed when severe breakouts damage the skin’s deeper layers, causing it to lose collagen and fat that keep the surface smooth. This leaves behind sunken areas known as atrophic scars, which look like pits or dents.
Picture your skin as a supportive framework made of collagen and elastin fibers beneath the surface. During bad acne, especially cystic or inflammatory types, white blood cells rush in to fight bacteria, oil, and dead skin clogging the pores. This defense creates strong inflammation that breaks down the pore walls. The contents spill into nearby tissue, harming the dermis, the skin’s middle layer.
As the body heals, enzymes from this response chew up too much collagen. Without enough of this protein to rebuild, the skin cannot bounce back to its even level. The spot collapses inward, forming a depression. Subcutaneous fat under the skin can also get destroyed, making the dip even more noticeable.
Not every acne spot scars this way. Mild pimples often heal without issue. Depressed scars happen more with deep nodules or cysts that burrow far into the skin. Repeated breakouts in the same area pile on damage, worsening the problem. Picking or squeezing pimples adds extra trauma, tearing more tissue and boosting scar risk.
Genetics play a part too. Some people produce less collagen naturally or heal unevenly due to family traits. Oily skin or delayed treatment lets inflammation drag on, increasing chances of these pits. Types of depressed scars include ice pick scars, which are narrow and deep like tiny holes; boxcar scars, with sharp edges and flat bottoms; and rolling scars, which give a wavy, uneven feel.
Over time, these scars stay because the skin lacks the support to fill in. Sun exposure or aging can make them look worse by reducing elasticity further. Early acne care helps prevent this by cutting inflammation short.
